Forms 1 and 2 must be completed for all athletes competing at CWOSSA. These forms must be handed in at Registration the Day of the Meet.

Form 3 and 4 must be completed for athletes advancing to OFSAA Cross Country Championships. These forms must be handed in at the CWOSSA Meet when athletes are declared for OFSAA. Athletes can not be registered in the OFSAA Meet without the Forms. Coaches with teams or individuals who they feel will qualify for OFSAA should bring competed forms to CWOSSA.

Form 3 "OFSAA Supervision Form" requires the Principal of the athletes school signature.
